Ukraine officially passes on new standards for HIV treatment.
The Ministry of Health of Ukraine approved a new clinical protocol for the treatment and prevention of HIV infection. These are the latest recommendations from the World Health Organization, which also include the public health aspect. Henceforth, anyone living with HIV, irrespective of the amount of the virus in the blood, has the right to receive free life-long treatment.
On June 5, 2019, the Ministry of Health of Ukraine by Order No. 1292 approved a new clinical protocol for the use of antiretroviral drugs for the treatment and prevention of HIV infection. This order formally approved WHO recommendations for treatment in 2018 and testing recommendations for 2016.
Ukraine is the first of all the countries of Eastern Europe and Central Asia to officially move to new standards for HIV treatment.
From now on, the treatment of infection will not only be more effective, more convenient, but also more affordable. According to the new approach, the drug treatment with dolategravure will be used as the main treatment regimen. With this active ingredient, a combined drug was developed that produces 9 companies. Due to this, the cost of one dose of the drug was cheaper by 6 times and now for Ukraine it costs 5 UAH 40 kopecks. The innovative combination drug combines several active ingredients, and therefore, the patient can take only one tablet daily.
The new protocol suggests that HIV testing can now only be made by two quick tests from different manufacturers. This rule will allow the person to be registered faster, and therefore, to start treatment for HIV more quickly and achieve the goals of the UNAIDS Global Strategy 90-90-90.
In addition, the new protocol allows the use of antiretroviral therapy for the prophylaxis of HIV infection. Preparations PrEP (pre-exposure prophylaxis), which last year were in Ukraine, are recommended for risk groups:
people in discordant pairs (in case one partner is HIV-positive and the other is not);
men practicing sex with men;
people who inject drugs;
people who provide sexual services for rewards.
The greatest innovation of the new protocol is the introduction of a public health approach for the treatment and prevention of HIV. It suggests that health care services for the detection, prevention and treatment of HIV will be provided in a comprehensive manner. Approaches will be simplified and standardized, and access of Ukrainians to quality medical services will improve.
The main elements of the public health approach in the treatment of HIV are:
use of simplified forms of medicines;
widespread use of fixed dose combination for first-line treatment in adults, adolescents and children;
provision of medical care and medicines to patients at the place of provision of medical services free of charge;
decentralization and integration of services, including redistribution of responsibilities and application of a simplified approach to clinical monitoring.

We remind that on April 28, 2017, the Ministry of Health of Ukraine issued an order No. 1422, which allows Ukrainian doctors to use international clinical protocols in their work. Health authorities can use such protocols in their work after approval by the internal order.
